Registering with the practice
You will need to come to the surgery to complete the registration process where you will be asked to complete a medical questionnaire.
You will not be registered until you have signed the application form and provided a minimum of 2 identity documents, one of which must show your address. Acceptable forms of identity for registration purposes are: Valid Current Passport, Photo Driving Licence, Household Utility Bills, Bank Statements
Please note that until such time as these documents are provided you will only be seen on a temporary basis and will not be fully registered with the practice.
Once you are registered with us, your electronic medical record will be available however it may take up to 6 weeks for us to receive your paper records.
Registering as a Temporary Patient
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can register as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
What information do I need?
Try to have the following information with you when you see your temporary GP for the first time:
- details of any medical conditions you have
- details of medical conditions you've had in the past
- the name of any medicines you're currently taking
- details of anything you're allergic to
- contact details for your permanent or previous GP
- NHS number
Please let us know if you have ever served in the armed forces as we will record this on your medical records and it may be helpful in your future care to be able to access specific services.
