We currently have Medical Students attached to the practice.  If you see one of the students you will also see one of the Doctors during your consultation.  This will obviously mean you will have a longer appointment.

We have recently welcomed a new lady doctor who is providing regular surgeries for us while Dr Surridge is on Maternity Leave.

Registering with the practice

filesYou will need to come to the surgery to complete the registration process - you will need to bring two forms of evidence of identity. 

You will be asked to complete a medical questionnaire. This is because it can take up to 6 weeks for us to recieve your medical records from your previous GP surgery.

PLEASE NOTE

YOU WILL NOT BE REGISTERED UNTIL YOU HAVE  SIGNED THE APPLICATION FORM, AND PROVIDED A MINIMUM OF 2 FORMS OF EVIDENCE OF IDENTITY AND LEGAL RESIDENCE IN THE UK.  

(acceptable forms of identity for registration purposes are:    Valid Current Passport, Photo Driving Licence, Household Utility Bills, Bank Statements)
